This year I am celebrating my 30th Anniversary in practicing cosmetic dentistry in Honolulu, Hawaii. To express my appreciation for your continued support through the years, I am giving away a smile makeover for a deserving Hawaii resident, who is 18 years or older, and who has given back to their community. If you are this special someone, we encourage you to submit your entry by emailing [email protected], with the information required below.

  4. Tell us in 200 words or less why you deserve to win a Smile Makeover
  5. Attach 4 photos:

(Photo 1) a smile photo of your full face

(Photo 2) a close-up photo of your full smile

(Photo 3 & 4) picture of your smile from the right and left side at an angle (so we can see the sides)
